This is a political candidate biography and questionnaire form. A letter with instructions for candidates in the November 2012 election will be sent out on Friday, Sept. 21. Deadline to complete this form is 5 p.m. Monday, Oct. 8. Please be concise in your answers. Overly long responses will be edited. Answer each question. Do not upload campaign literature.
